What to Bring to Your Appointment
To ensure a smooth and efficient visit, please bring the following:
- A referral slip and any relevant x-rays, if applicable.
- A comprehensive list of all medications you are currently taking.
- Your medical or dental insurance information for claim submissions.
Important Information
- Patients under the age of 18 must be accompanied by a parent or guardian for any surgical appointments.
- For patients undergoing IV anesthesia or sedation, a review of your medical history and a physical examination will be conducted during the appointment.
- Pre-Surgery Preparation: Do not eat or drink anything for at least eight hours before surgery. Additionally, arrange for an adult to drive you home afterward.
- If you have a medical condition that may impact your surgery, such as diabetes, high blood pressure, or other concerns, please inform the office in advance.

X-Rays
If your dentist or physician has taken x-rays, you may request a copy to bring to your appointment. Alternatively, you can have them sent directly. If additional x-rays are needed, they can be taken at our facility.
